Planned Parenthood of Southern New England renovates patient-centered medical home in Hartford

Hartford, CT - On Monday, October 23, 2017 Planned Parenthood of Southern New England (PPSNE) opened its doors to its new state-of-the-art health center in Hartford, Conn. PPSNE is at the same address – 1229 Albany Avenue in the North End of Hartford – but relocated its health center and education center across the hall to a completely renovated and redesigned space. The move comes on the heels of the health center receiving National Committee for Quality Assurance Patient-Centered Medical Home recognition for using evidence-based, patient-centered processes that focus on highly coordinated care and long?term, participative relationships.

“We are thrilled to expand our services, community involvement and educational programming in the North End of Hartford to serve the community well into the future,” said Amanda Skinner, President and CEO of Planned Parenthood of Southern New England. “We are improving health outcomes in the Hartford community by revolutionizing access to affordable, comprehensive health care under one roof. Our patients are getting the care they need by receiving primary care and reproductive health care services from our expert providers.”

The new health center is nearly 5,000 square feet – double that of the previous location – with six exam rooms and three counseling rooms. Last year, PPSNE provided high-quality health care services to nearly 4,500 people in Hartford. The Hartford health center has free and accessible off-street parking, and for convenience, online, walk-in and same day appointments are available. PPSNE provides the full spectrum of reproductive health care services, as well as treating conditions such as blood pressure, diabetes, depression, cholesterol and asthma. Every month, more than 100 primary care appointments are made in Hartford.

The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduce costs. Medical homes foster ongoing partnerships between patients and their personal clinicians, instead of approaching care as the sum of episodic office visits. Each patient’s care is overseen by clinician-led care teams that coordinate treatment across the health care system. Research shows that medical homes can lead to higher quality and lower costs, and can improve patient and provider reported experiences of care.

The Hartford health center was named after Judy Tabar, who served as PPSNE President and CEO for 20 years before retiring this past spring. Tabar’s legacy is one of innovation, which is evident in the breadth and quality of services provided in the North End of Hartford. A special grand opening celebration to honor Tabar is scheduled in the coming the months.

Through the $22 million Building Futures Campaign, the Hartford location, as well as five other health centers in Connecticut and Rhode Island, were upgraded, redesigned, or relocated. These long-term investments best position PPSNE to continue serving its mission and communities for many years to come.

Planned Parenthood is a trusted, local resource for convenient and confidential services for women, men and young people, including STD/STI testing, all FDA approved birth control methods, pregnancy testing and counseling, emergency contraception, and HIV testing, counseling, and prevention (Pre-Exposure Prophylaxis [PrEP], a daily pill that helps prevent HIV by more than 90 percent) and more.

The Hartford health center is open Monday – Thursday 8:30 a.m. – 6:45 p.m.; Friday 10 a.m. – 4:15 p.m.; and Saturday 10 a.m. – 1:30 p.m.
